Fremont County Public Health Agency, 719-276-7450, 201 N. 6th Street. Canon City CO 81212. Office Hours from 7:30 - 4:30. Mon - Thurs. Must have valid photo ID.$17.75 for the 1st copy and $10 for any additional copies at the time of request
