The following forms are optional. You can drop off these forms at your childs school starting August 2, 2022. Each school's office hours will be 8am-3pm. In-person supports will be available for anyone who might still need help with registration.
School Based Health Clinic Permission forms -
In order for your child to receive treatment at The Pulse, our school-based health center in the Cañon City School District, you will need to fill out the forms contained in the links below. If you do not have a PaySchools account set up, please obtain your student(s) 10 digit STUDENT NUMBER from your school or by calling 719-276-5813. Once you receive this number, setting up an account and applying for meals is simple.
Free and Reduced Meal Applications
Free and Reduced Application are required for all households this upcoming year 22-23. COVID is over and we are back to traditional meal service. Children who do not qualify for Free and Reduced will have to pay for their meals.
Campus Portal Account - If you do not have a Campus Portal account, you can sign up for one here. Proof of ID is required. Sign up for Campus Portal . Campus Portal provides real-time access to announcements, assignments, attendance, grades, schedules throughout the year. Please fill out and submit the following form.
